As a Patient Care Coordinator, I serve as a liaison for patients by connecting them to different departments, clinical staff, and resources. I coordinate various aspects of patient care and provide quality assistance to ensure smooth transition between care settings.The function of my role is to:•Manage a high call volume of 30-50 calls daily from referring physicians, patients, family members and others wishing to access care at SCCA•Improve patient access by managing the schedules of 12 different imaging and medical procedures at the Seattle Cancer Care Alliance.•Request medical records from referring physicians and outside institutions, reviews them for content and completeness, and organizes them into designated categories for clinical review.•Collect and enter patient demographic and insurance information into registration databases.•Reduce scheduling errors by cross checking schedules between multiple scheduling systems.•Maintain department work queues ensuring that referrals are completed in a timely and complete manner•Assist in coverage of work queues and related positions locally and across community sites.•Receive and responds to patient requests and feedback
