Provide strategic leadership that provides continuity and coordinates Common Ground Health activities across various departments, programs and projects. • Supervise assigned programs and Program Managers to ensure timely and effective program operations and to communicate and document all aspects of programming required to inform agency budgets, proposals and reports. • Lead internal conversations to identify opportunities for agency performance improvement and, based on these conversations, identify and implement appropriate corrective action to avoid/respond to deviations from agency workplan. • Provide leadership and strategic input for program, project and partnership strategies that affirm Common Ground Health\'s role as the region’s convener of, and catalyst and champion for, collaborative community health improvement. • Provide technical assistance and support to the NYS Department of Health’s Population Health Improvement Program (PHIP) and to PHIP contractors by sharing health improvement best practices and – when needed – through local, state and national advocacy. • Advance Common Ground Health and its short and long-term goals in local, state and national forums, boards and collaborative groups focused on community health improvement. • Serve as trusted/reliable resource for consumer and community engagement and disparities elimination for internal and external stakeholders while providing strategic leadership and support to Common Ground Health\'s African American Health Coalition, Latino Health Coalition and Partnership on the Uninsured. • Work across multi-sectorial/stakeholder internal and external teams – particularly, those involved in the regional High Blood Pressure Collaborative – to bring expertise related to the Determinants of Health to agency activities and those of collaborative partners.
