Passionate about Getting Real Work Done in improving health outcomes for the groups that need it most. Siftwell was founded with the fundamental belief that for organizations such as Medicaid plans managing the care of our neediest and most complex populations, there is a lack of appropriate tools to help member-facing teams know who needs their support the most and what to do about it. Because of that, we harnessed our MCO operating expertise to build a product that helps these community-oriented organizations better understand their populations\' needs, and the Pop Health & Quality teams inside them better prioritize their outreach and interventions on the members that need it most. We’re using the latest in causal and explainable AI to help teams understand what precise, micro-efforts to aim at what members and how to allocate resources to improve member health.
