Abstract and assign for educational purposes ICD-10-CM, CPT, and HCPCS Level II codes from real worldoutpatient medical charts in multiple specialties: orthopedics, general surgery, ENT, plastics, and E/M, and pain management. Apply official coding guidelines, NCCI edits, and payer policies to ensure accuracy, compliance, and optimal reimbursement alignment. Review and validate clinical documentation for completeness and accuracy; identify discrepancies (if any), and escalate for clarification. Analyze coding trends and patterns to support root-cause analysis, reporting issues to TeamLeads for process improvement. Utilize encoder software, Codify, CMS resources, CPT Assistant, and AHA Coding Clinic to research, validate, and resolve complex coding questions, to validate code assignment and resolve coding ambiguities. Maintain up-to-date knowledge of medical terminology, anatomy, physiology, pharmacology, diseases processes, and coding updates to ensure consistent code accuracy.
