Spearheaded the successful development and implementation of Innovative Health and Behavioral Health Integration (BHI) initiatives within The Harris Center. The Director facilitates the Certified Community Behavioral Health Clinics (CCBCH) care delivery and primary health behavioral health integration program in collaboration with health-related partnerships with universities, federally qualified health systems, and local hospitals and outpatient practices. Developed unified health and behavioral health strategy for integrated health- behavioral health home and patient centered medical home practices – reduced cost of care for MCO members significantly and received annual shared savings. Worked with managed care organizations to create value-based shared savings program for specialty needs population (chronic health conditions and severe mental ill) that resulted in improvement of patient experiences, reductions in preventable hospital visits, and significant reductions in cost of hospital care. Spearheaded Program Pilot that was one of three nationally and is now a promising practice model national for chronic care and complex conditions in partnership with managed care organizations – reducing the cost of care and improving health outcomes that was nationally recognized. Work with teams to create population health and risk stratification tools, metrics and best practices that positions the Harris Center to access funding sources for health services in light of Co-chair of mental health sub-committee of the Harris County Health Equity Collective – addressing health disparities, health equity, systemic challenges that lead to poor health/behavioral health outcomes.
