Nationally recognized through the Health Service Journal.· Established 12 Primary Care Networks that are the building blocks of the delivering primary and community integrated care.· Developed the system wide operating model for the development of a Health and Social Care Alliance covering a population of over patients.· The Provider Alliances is the central vehicle to deliver the outcomes within the emerging Integrated Care System.· Established case for change for a day hospital to alleviate demand pressures on local Acute trusts.· Leading on core values and behavioural expectations to secure a positive and engaged culture between the organisation, its patient’s staff and external stakeholders.
