ABOUT MOSTAFA ALQEMARY

As a dual Board-Certified Pharmacotherapy Specialist (BCPS) and Ambulatory Care Pharmacist (BCACP), I bring over a decade of progressive clinical pharmacy experience across diverse healthcare settings—including hospital, community, ambulatory care, and interdisciplinary teams. Licensed in four countries, I currently serve as an Ambulatory Care Clinical Pharmacist at AltaMed Health Services, where I manage complex medication regimens for patients with chronic conditions such as diabetes, hypertension, hyperlipidemia, heart failure, asthma, and COPD. Additionally, I also lead the Enhanced Medication Management Program (EMM), which aims at providing targeted medication interventions for patients with the most complex medication regimens to reduce ADRs & improve outcomes.My clinical approach combines evidence-based medicine with a patient-centered mindset. I routinely collaborate with providers to optimize medication therapy, enhance adherence, reduce adverse drug events, and improve health outcomes. I am well-versed in utilizing EMR platforms like EPIC, Meditech, and Pyxis, and I take pride in leading medication therapy management (MTM), comprehensive medication reviews (CMRs), and targeted interventions tailored to individual needs.In parallel with my clinical work, I’ve embraced emerging technologies in healthcare. For over two years, I’ve been leveraging AI-powered tools—most notably Surveyor Health—to identify high-risk patients, stratify medication regimens, and reduce the incidence of drug-related problems. This intersection of clinical care and advanced analytics reflects my forward-thinking approach and commitment to innovation.My educational background includes a Doctor of Pharmacy degree and additional specializations in AI in Healthcare (Stanford University), Health Informatics (Johns Hopkins University), and Value-Based Care (University of Houston). These credentials reinforce my passion for harnessing technology to elevate patient care.Whether in clinical practice or digital health, I’m driven by one mission: empowering patients through safer, smarter, and more effective medication use.
