Supports the Partnership for Quality Care Innovation (PQCI) Team by coordinating meetings, schedules, activities, communications, and other administrative tasks.• Assists and monitors assigned practices in the implementation of QI initiatives and processes PDSA cycles.• Works directly with lead physicians and their staff to address issues related to quality improvement and care processes within their practices. • Gathers appropriate data from QI reports, audits, and/or outside sources and shares with physicians and their practice staff.• Works directly with practice transformation coaches and their assigned providers to assist them in the development of disease management initiatives and protocols. • Provides staff training as needed to implement QI processes and initiatives.
