My projects improve patient care by implementing prescribed evidence-based care management and practice transformation strategies. I work with Federally Qualified Health Centers (FQHCs), physicians and other patient care providers to support them to meet quality performance objectives through data collection, data analysis, process outcome evaluation, and other PDSA rapid cycle quality improvement methods. I work independently and collaboratively in carrying out the goals of the project, working alongside practices, other consultants, health plan divisions and departments, government, and community collaborators.
