To promote and safeguard the welfare of vulnerable adults.Enable people to experience personalised, integrated care and support to maintain their independence and wellbeing with a real focus on vision and inclusion to support people in attaining the outcomes they want and need. Support individuals, their families and carers, to understand and manage risk whilst participating in the life of their community and wider society. To ensure that people, their families and carers are able to contribute to their assessment, are heard and have control over their identified outcomes through effective joint working, good communication and a positive working relationship based on trust and respect. Undertake re-assessments and reviews, in accordance with Care Act eligibility criteria and national legislative requirements, which will include: review of the eligibility determination, the social work assessment, risk assessment, the care and support plan and the resources allocated to the person. Care and Support plans must also maximize the use of Assisted Technology and Occupational Therapy. To support customers and carers in identifying outcomes and options to achieve their aspirations and build their resilience - including identifying their existing personal strengths and the support available from families, voluntary agencies, community resources and health colleagues.Focuses on making the best use of available community resources and assistive technology to enable people to have independence. Where appropriate work collaboratively with colleagues and partners of the Council to ensure people have accurate financial information relating to the Council’s paying for care policy.Participates in Decision Support Tool meetings, CHCP meetings, MDT,s, complete prompt sheets for Practice Surgery, Community DoL checklists and any other documentation/meetings as directed by the leadership team.
