Manager of two cohort teams consisting between 5-12 team members depending on cohort assignment/contracts devoted to taking care of our designated patient population which consists of anywhere between patients. My team consists of RNs, CNAs/MAs, Pharm D/CPPs and we currently service the Wake area including 9 separate clinics including EMR expertise in Allscripts, Advanced MD, Practice Fusion, Amazing Charts and ECW. Our goals involve scheduling target patients in need utilizing care gap data and risk association. Once scheduled patient meet with our clinic pharmacist and in collaboration with the patient schedules subsequent follow ups to address and work towards healthcare goals. I manage day to day team operations and am responsible for health care gap closures, team success, clinic communications, weekly team meetings, ED Discharge assignments, clinical lead schedules, documentation processes, patient satisfaction, time card approvals, workflow processes, team scheduling, etc.
