Identify and prioritize individual needs and problems.• Document client progress toward treatment plan goals and objectives in individual progress notes.• Develop solid transition and discharge plans and summaries for assigned caseload.• Prepare clinical documentation accurately and within agency time frames.• Intervene during crisis intervention situations.• Point of contact for families regarding clinical care• Participate in the agency’s Quality Management system, Orientation, staff meetings, and in-service training.• Provide accurate assessment and referral when appropriate.• Attend staff meetings, supervisory conferences, training, and other activities, which ensure the effective and efficient functioning of clinical operations.• Adhere to organizational standards and policies and procedures, code of ethics, legal aspects of clinical practice, professional standards, the duty to warn, abuse and neglect reporting policies and procedures and exercise professionalism at all times.• Ability to assess for safety and provide appropriate interventions according to AMFM Policy and Procedure.
