Responsible for providing high quality, supportive end-of-life spiritual and bereavement care services to hospice and palliative care patients and families. Provide pastoral care in homes, skilled nursing facilities, hospital setting, hospice in-patient unit and adult living communities. Conduct initial and ongoing assessments and care planning for hospice patients’ and family members bereavement, spiritual care, and general counseling needs related to the dying process. Implement plan of care acknowledging and respecting the patient’s and family’s beliefs, traditions, culture and values related to life’s meaning, suffering and loss.
