I currently collaborate with a team to provide out-of-hours emergency response for young peopleand adults who reside within Leeds, Sheffield, Worcester, Wakefield, and Bradford. We analyse calltimes and work performances to provide person centred emergency response and immediatesolution to young person or adult who has social care difficulties that are causing a serious risk orconcern that cannot wait until the start of the next working day. My duties entail working collaboratively with carers on field to achieve the best outcomes for the individual, we effectively prioritise and manage workload. We decide on what to do and advise on the use of appropriate social care services and resources in an event of emergency. We investigate and assess the need for social care intervention during out of normal office hours and communicate effectively with individuals, carers, other agencies, and professionals to support the delivery of a coordinated response to care.
