Working alongside a team of 9 General Practitioners and 4 Physician Associates within a busy practice catering to patients. • Establishing and maintaining long-term rapport and continuity of care for all patients.• Being the first point of care for patients who experience physical and mental health problems.• Taking comprehensive clinical histories and triaging patients appropriately based on the urgency of the presenting complaint.• Conducting 10 weekly sessions, undertaking 12-15 patient telephone consultations in the morning clinic, home visits and 8-10 face to face appointments in the afternoon. • Performing clinical examinations• Working with patients to formulate a shared management plan which can include explaining all the possible management options such as medications, counselling, alternative local services, letters for advice, discussing with on-call specialty doctors and appropriate and timely referral to secondary care. • Completing Medication reviews and formulating medication plans. • Filing blood test results• End of life discussions, advanced care planning, preemptive prescribing • Conducting audits to identify gaps within service delivery and patient outcomes and creating a plan to improve service as part of the Quality and Outcomes targets.• Attending and engaging in clinical team meetings on a weekly basis. • Running Flu and COVID-19 vaccination clinics.• Teaching Medical and Physician Associate student who come to the surgery on clinical placements. • I also conduct three sessions performing general medical responsibilities at the stroke rehabilitation unit, for individuals who are unable to manage at home as a result of an orthopaedic condition or following a stroke.• Conducting cancer care reviews which includes but is not restricted to discussing their diagnosis, treatment, reviewing medications, providing a holistic approach of care for the patient. • Conducting learning disability consultations and exam
