   In my 5th and 6th year of the Doctor of Pharmacy (PharmD) program, I transitioned into a more advanced clinical internship role at Ruby hall clinic hinjewadi, where I continue to work as a Clinical Pharmacist Intern. With increased responsibility and independence, I actively contribute to patient care across inpatient and outpatient settings.Key responsibilities include:1. Performing pharmacotherapeutic evaluations and providing evidence-based recommendations during ward rounds.2. Monitoring treatment regimens, including high-risk and chronic care patients, with a focus on dose adjustments, ADR monitoring, and therapy optimization.3. Participating in chemotherapy preparation and dilution under supervision in the oncology unit.4. Visiting OPDs (Outpatient Departments) to support medication counseling and patient education initiatives.5. Maintaining and updating patient Electronic Medical Records (EMRs) with medication histories, clinical notes, and interventions.6. Conducting discharge counseling and ensuring medication reconciliation to promote continuity of care.7. Actively participating in ward round shifts as part of the clinical team.8. Reporting and documenting adverse drug reactions (ADRs) and assisting in pharmacovigilance activities.9. Presenting clinical cases, journal clubs, and drug reviews to the healthcare team.10.
