The COVID pandemic has acutely exposed the relationship between inequality and health that has always existed in underserved communities. The shift from volume to value-based care exposes inefficiencies and create opportunities to improve health disparities for marginalized communities. The health care system alone cannot end health disparities rooted in longstanding inequities in the social, economic, and environmental conditions necessary for a healthy life. This is a difficult population to engage; current solutions ineffectively target high risk cohorts and fail to provide substantive health system cost savings.Trestle Health and Housing (Trestle) has created HomeSight - an innovative, value-based, population health management (PHM) model to address health disparities in underserved populations. The major driver of U.S. health costs - chronic illness, depression and “gaps in care” – are significantly higher in these populations as opposed to the general population. HomeSight is designed to address both these clinical realities and provide resources to help residents overcome historically stubborn social determinants as obstacles to their health and well-being. Capabilities to measure, manage and coordinate medical and non-medical care are delivered where residents with complex chronic illnesses are most likely to accept them – in their home. Learn more at
