I\'m a visionary and innovative healthcare executive with in-depth knowledge of current and upcoming trends for future health care systems. My expertise encompasses Care Coordination, Transitional Care, Case Management and Specialty Care Program Development in post-acute care environments. I specialize in innovative transitional care and advanced clinical program design and implementation in skilled nursing facilities. I\'m a noted Subject Matter Expert in transitional heart failure post-acute care program development and implementation with proven readmission reduction. I’ve been in nursing for 25 years. I started my career as a Certified Nurses Aid, then increased my clinical scope and responsibilities to a Licensed Practical Nurse, a Registered Nurse to Day Supervisor, Director of Nurses, Regional Nurse/Director of Transitional Care then to Corporate Director of Clinical Services, Chief Clinical Officer and to my current role as Director of Case Management. I am further honing my clinical leadership skills through pursuit of an Advanced Practice Registered Nurse (APRN) post graduate degree. I recently completed my Master’s Degree in Care Coordination. I also possess a Bachelor’s Degree with a concentration in Care Coordination.As a Nurse and clinical leader, my personable style has brought comfort and peace to many patients and families facing difficult health choices and illnesses. My mission is to support quality care through enhanced transitional care delivery; patient and caregiver education & advocacy while reducing readmission, cost and managing LOS for the best quality outcomes possible in the post-acute clinical care setting while fostering partnerships and building new business.
