I have demonstrated success in the development and implementation of innovative community-driven program models including over 20 years in the design and use of community coalitions to address health needs. My achievements include establishing one of the most successful community-led collaborative structures of its type in the nation - an engine for the effective delivery of hospital community benefit using a \"collective impact\" approach. I was responsible for overseeing the planning and launch of a nationally recognized health coverage model that is unique to our community. Currently a stand-alone nonprofit, our coverage product has served thousands of local residents addressing both health access and individual health improvement.I was one of a group of founders of Communities Joined in Action (CJA) a national organization linking coalition-led work throughout the country along with emergent best practices in community access. CJA embraced the Bureau of Primary Care\'s national strategy of 100% access and 0 disparities and identified 8 critical activities (programs) fundamental to improving access and reducing disparities. These activities can serve as the basis for high impact community benefit redesign.As a community benefit practitioner in one of our nation\'s largest health systems, I fostered use of a standardized Community Health Needs Assessment and Implementation Plan process; created a Playbook to successfully reach and enroll uninsured populations; fostered the expanded use of Community Health Workers along with evidence based approaches to measure their impact; and, designed community outreach strategies that targeted vulnerable populations using \"heat maps\" or \"hot-spotter\" tools.This history - along with over 14 years in public policy - have given me the skills and knowledge to produce results for my clients during this period of health delivery change and innovation.

Senior level strategic consultant with over 30 years of experience in community organizing, public policy and health improvement strategies. Consulting experience spans 20+ years and includes both government and private sectors. I have significant expertise in hospital community benefit, safety net services to vulnerable populations, collective impact approaches, the use of collaborative structures, and community care coordination to target social determinants. As an independent consultant I specialize in redesign of community benefit investment portfolios as well as community strategies and tactics to foster health improvement.
