Provide person-centered support to individuals with developmental disabilities, mental health needs, and complex behavioral or medical conditions in residential and community settings.Assist individuals with daily living activities, including personal hygiene, meal preparation, mobility support, medication reminders, and household tasks to promote independence and dignity.Implement Individual Service Plans (ISPs), Behavior Support Plans (BSPs), and risk management strategies in alignment with 245D Minnesota DHS standards.Observe, document, and report changes in behavior, health, or safety, ensuring accurate and timely completion of progress notes, incident reports, and communication logs.Support individuals in community engagement activities, employment goals, transportation, and social participation to enhance quality of life.Maintain a safe and respectful environment by following emergency procedures, de-escalation techniques, and positive behavioral supports.Collaborate with interdisciplinary teams, including nurses, therapists, case managers, and supervisors, to ensure coordinated and effective care.Administer or assist with medications when delegated, following Sevita protocols and Minnesota 245D medication administration requirements.Advocate for individual rights, preferences, and goals, promoting self-determination and empowerment in all interactions.Provide emotional support, active listening, and crisis intervention as needed with professionalism and compassion.
