I am the Executive Director of Population Health at MedStar Shah Medical Group, a MedStar Health entity, where I lead enterprise-wide transformation across ambulatory, hospital, and post-acute settings. My work centers on designing and implementing sustainable, tech-enabled care models that improve outcomes, reduce costs, and support the transition to value-based care.With over 15 years in healthcare leadership, I’ve led systemwide strategies aligned with CMS innovation models including MDPCP, AHEAD, TCOC, and multiple Accountable Care Organization (ACO) programs. I bring deep expertise in operationalizing value-based care frameworks—integrating clinical workflows, payment reform, and team-based care coordination for high-risk and underserved populations.My work spans utilization review, digital health adoption, health equity initiatives, and AI-driven patient engagement. I lead cross-sector collaboration with clinical, operational, and technology teams—bridging policy and frontline practice to deliver care models that are scalable, sustainable, and equitable.I also contribute to broader system initiatives focused on ambulatory quality and safety, clinical documentation improvement (CDI), infection prevention, and patient experience transformation. These efforts are rooted in operational excellence and frontline execution—ensuring care delivery meets safety, satisfaction, and regulatory standards.I have had the privilege of contributing at the state and national level through participation in programs such as the CMS AHEAD Primary Care Advisory Council, the CMS Partnership for Quality Measurement (PQM) pre-rulemaking process, and the Mid-Atlantic QIN-QIO Executive Leadership Council.Whether supporting primary care redesign, aligning ACO strategy, or embedding social determinants of health into care pathways and platforms, I focus on initiatives that work in real-world environments.I enjoy contributing to conversations at the intersection of technology, innovation, and care redesign.
