The Facility Telehealth Coordinator (FTC) manages care across the health care continuum, working closely with primary care providers and other health care professionals and team members, other clinics, internal services and community agencies in a facility level position, reporting to the ACOS clinical informatics, with coordination and input from the VISN Telehealth Manager.The FTC is responsible for overall planning, coordination, implementation, management, and evaluation of clinical applications of telehealth within the facility organization, with particular emphasis on improving access to veterans identified as \"rural\" and \"highly rural\".The FTC works closely with network and facility leaders and staff to incorporate new concepts and programs into extant systems and provides education and training to all facility staff impacted by program, role, or technology changes. Matrix lines of authority ensure that VISN 21 and VHA goals and strategic directions are fully supported by telehealth programming through collaborative efforts with facility and network leaders.The FTC is knowledgeable of overall facility and network goals, missions and priorities to ensure that the telehealth programming does not impact adversely upon other programs or resources, knowledgeable of funding sources and methodologies including Office of Rural Health, the area Rural Health Resource Center, VERA, third party collections, sharing agreement revenues, contracts and enhanced use revenue, with awareness of management discussions, decisions, and policy-making concerning Telehealth and rural health; consults and advises the Chief of Staff, Facility Director, and Chief Medical information Officer on all relating matters. The FTC integrates telehealth program planning within facility operations as a whole to ensure goals and priorities are complementary and in congruence with the overall VISN telehealth programs.
