Provide services from a trauma informed care perspective, to a diverse population of at risk Transitional Age Youth (14-22) and their families; including but not limited to military families, single parent/divorces families & families involved in the justice system,•Participate in therapeutic milieu•Facilitate/Create groups, ranging from, but not limited to: DBT & CBT groups with Fellows from UCSD Adolescent Psychiatric Fellowship Program, Grief Therapy, Anger Management, Coping Skills, Music Therapy, Art Therapy, Self-Awareness, Overcoming Addiction with CBT, Social Skills, Communication Skills•Individual, family, & group therapy utilizing: CBT, TF-CBT, MI, DBT, Solution Focused Therapy, & Behavioral Therapy•Designated Intensive Care Coordinator for Foster Youth in compliance with KTA requirements•Participate in treatment team meetings, Intensive Care Coordination meetings, Individualized Education Plan meetings, & clinical consultations•Chart audits for quality assurance•Case management services to students & families when necessary•Case assessments to determine needs of student, with continued reassessment to ensure interventions/client plan and diagnoses remain effective & accurate•Consultation to relatives, families, and other care providers to ensure collaborative treatment and/or intensive care coordination•Prepare case history information & develop client plans •Maintain appropriate records for history & health, including electronic health records & Medi-Cal documentation-(Anasazi)•Evaluate medical records for accurate & complete documentation of medical necessity, treatment modalities & appropriate level of care, including complete psychiatric assessments•Prepare therapeutic & acute discharge for clients, with overview of mental status, progress, referrals/resources, & prognosis•Crisis intervention (telephone) for current/previous clients & their families as needed; outsource appropriately
