Provides clinical case management services for chronically homeless Veterans who are eligible for a HUD Section 8 voucher; promotes the Housing First Model, a low-barrier supportive housing model that prioritizes safe and stable housing- Assesses and treats complicated psychosocial challenges impairing Veterans’ daily functioning and overall quality of life- Facilitates multidisciplinary treatment team meetings with nursing and psychiatric care professionals to provide wrap-around services to address comorbid conditions impacting the Veteran population- Consults and educates Veterans on available community resources, VA benefits and specialty programs, advance directives, and completes referrals for additional VA, government, and community programs- Works collaboratively with Veterans to assess acuity based on Veteran’s ability to successfully complete ADLs and IADLs; supports Veteran’s transition into appropriate clinical level of care- Serves as a liaison between the Veteran and local public housing authority (PHA) to ensure successful and accurate completion of all entry and recertification paperwork to maintain Veteran’s subsidized housing- Formulates evidence-based individual service plans to promote housing stability for Veterans- Project-Based Team Lead, effective August 2024.

ABOUT TYLAH STEVENS

Tylah Stevens is a clinically licensed social worker in the District of Columbia and Maryland. An alumna of both the George Washington University and the Howard University School of Social Work, Tylah has over 10 years of experience in the social work and human services fields. Tylah has acquired a diverse range of skills through working in government and community-based organizations with different populations that include families impacted by the child welfare system, individuals with mental health diagnoses, members of the LGBTQIA+ community, survivors of trauma, and Veterans. Trained in motivational interviewing and solution-focused therapy, among other modalities, Tylah is committed to achieving positive outcomes for clients and motivating others to do the same. Through her work with marginalized communities, she is dedicated to empowering the next generation of clinically trained social workers to provide inclusive, ethical, and culturally competent care to all clients.
