i) Managed and led the team that coproduced, procured, implemented and now embedding the Maximising Wellbeing at Home service model (£19.6m per annum) enabling Clients to choose their Wellbeing Team, allocate their weekly hours, is fully compliant with the UNISON Ethical Care Charter, as well as ensuring Wellbeing Workers develop their knowledge and skills through a career pathway structure. Spend is now managed through a capitated budget.ii) Facilitated development of the Carer Implementation Plan that formed the framework for the procurement of the Maximising Wellbeing of Unpaid Carer Led the team that developed the business case for implementing a Proactive model of Care for Older Adults (£500k annual cost) in collaboration with NHS Acute Trust and Community colleagues that will reduce Emergency Department attendances by £1.3m per annum and emergency admissions by £7.5m.iv) Drafted a number of reports for Mayor and Cabinet.v) Initiated public consultations for a range of strategies.vi) Participated actively in the Healthier Communities select committee.
