I am thankful to be working with Remote Care Today by SHS as a care consultant in the field. Our main goal is to help seniors live a better quality of life, as well as benefit our clients who work directly with seniors. Our programs are beneficial to patients and to our clients, as well as Medicare in that they are preventative focused and designed to reduce the burden on the healthcare system.1. Chronic Care Management (CCM) & Remote Patient Care (RPM)- Medicare realizes practitioners don\'t have the time, resources or staff to know how their patients are doing between office visits. Our remote care solutions alert you to changes in vitals, as well as frequent verbal communications between our medical professionals and your patients between visits.Here are a few reasons why our CCM and RPM services are so valuable: Better care for patients/increased patient satisfaction Compliance with required Medicare CCM/RPM programs Increased monthly revenues for qualifying providersDecreased hospital readmissions No additional staffing needed, often relieving over-burdened staff.2. Our Remote Therapeutic Monitoring (RTM) program helps patients with improved cognition and memory, improved balance, fall prevention and relief from stress and anxiety.
