Work with partner agencies to identify and outreach eligibility participants.• Assist eligible frequent users with competing HAF/PHA/DSS application process• Assess each client’s needs and strengths and collaboratively develop their personal goals.• Arrange for identity cards and other documents necessary to assist clients to obtain all basic needs supports and other entitlements such as Medicaid, Supplemental Social Security, Social Security Disability, Supplemental Nutrition Assistance and appropriate housing subsidies etc.• Assist with budgeting, bill-paying, housekeeping, shopping and cooking.• Provide clinical support by helping to integrate mental health and addiction treatment and use of medical and wellness services.• Encourage social supports including involvement in peer activities and groups and the development of connections to community activities related to the tenant’s interest.• Connect client to job services and assist with employment alternatives such as volunteer work or steps towards achieving employability such as obtaining a driver’s license or completing vocational training.• Other duties as assigned
