We formed the Transformation Capital predecessor fund, LTP Fund I in 2016 after I co-led healthcare investing at Sequoia Capital and before that as a CEO/founder and operator with two successful exits (Picis - filed for IPO & sale to UnitedHealthGroup and Marquette - IPO and sale to GE). My partner Jared Kesselheim, MD/MBA, a Harvard/MGH-trained clinician but more importantly Jared led healthcare investing at Bain Capital Ventures with an 8 year career there and a stellar track record. We partnered with Leerink, the largest healthcare-focused investment bank which brings strong industry knowledge and connectivity to our platform. We were joined this year by Mike Dixon a general partner at Sequoia who I worked with for over four years there. Mike is one of the most successful digital health investors.    SWORD Health provides completely virtual physical therapy for preventive and rehabilitative musculoskeletal (MSK) health. MSK is the leading healthcare complaint area for working adults. SWORD\'s solution covers the 14 most common joint conditions by integrating bodily sensors with an AI-powered application that monitors the patient\'s exercises and gives them feedback to do the motions correctly and in the right order and repetitions. It is integrated with telehealth consultation from licensed physical therapists who check in from time to time with the patient. Employers are seeing savings and better outcomes and the patient has the convenience of therapy on demand in the home.
