Responsibilities include but are not limited to, Initiating and organizing client’s education and triaging. Proactively contacts members to provide wellness education and disease management information and services. Performs complex specialty related procedures. Educate Fellows in system EPIC use.Communicates and collaborates with clients’ multi-disciplinary team in health assessment and organization of patient confidential information. Contributes input and experience to quality and continuity of care improvement initiatives.
