Counsel on anticoagulation medications to ensure patients understand the purpose and importance of their anticoagulation• Execute medication reconciliation with newly admitted patients and patients being discharged to ensure they receive all their medications during their stay and that they understand what their old/new medications are treating• IV (intravenous) to PO (by mouth) conversion to improve patient care and cost effectiveness of treatment• Non-sterile compounding of oral medication stock bottles to enable accurate medication dosing for pediatric patients and to assist in medication administration for patients with feeding tubes• Complete technician tasks such as filling prescriptions, assisting nurses at the inpatient pharmacy window, and delivering medications to units throughout the hospital• Represent inpatient pharmacy to local high school outreach programs that aim to educate on careers in health care.
