Demonstrate a high level of accountability with communication reporting to Chief Operating Officer • Lead team of seven people in Home Care, Medicaid, Housing, and Clinical Assessment and Evaluation - managing personnel aspects of supervision, including hiring, training and evaluation of Managers. Completes performance appraisals as required.• Reviews and evaluates staff performance. Provides support and encouragement to managers in all aspects of their professional development for my direct reports and those their teams. Provides technical support and re-training as needed. • Work with providers to develop and maintain a professional relationship with staff, community, agencies, hospitals and nursing facilities• Maintains accurate current data and quality assurance program needed for statistical reports to Elder Affairs• Ensures Registered Nurses who conduct in-home assessments for Home Care consumers who are in need of assistance with personal care including: bathing, dressing, transfers, mobility and eating. The RNs, in collaboration with the Case Managers, determine the level of service required.• Ensures that consumers who have applied for, or are beneficiaries of MassHealth, our RN staff determine medical appropriateness for MassHealth payment of services such as nursing facility, adult day health and/or home and community based waiver programs.• Implement new services and programs such as new business opportunities or pilot programs approved by Elder Affairs • Participates and supervises the interdisciplinary review of all cases from intake and assessment through on-going case management. Ensures all case records meet regulatory and contractual requirement based on eligibility criteria and their specific service plan• In conjunction with manager’s review tracking systems to insure all case managers are meeting visit schedule, data entry and documentation standards.
