   The TIP (Transition to Independence Program) supports teenagers and young adults transitioning into adulthood while managing the additional complexities of mental illness. This intensive program helps bridge the gap between the youth and adult mental health systems. The Transition to Independence Program in Hamilton County is available to youth ages 16-22.The TIP (Transition to Independence Program) Case Manager is responsible for assisting clients and families in achieving stated goals and objectives, and maximizing independence and daily functioning through interventions and advocacy. Utilizing a best practice model, emphasis is placed on supporting completion of high school/vocational programming, seeking and retaining employment, and establishing self-sufficiency. The TIP Case Manager provides ethical and therapeutically appropriate evidenced based interventions to clients and families by utilizing both internal and external resources to positively impact every client’s life.
