Manages and supervises a team of seven (7) community health workers- Assists with developing and implementing the objectives, training outline, and workplan for the community health worker program model- Fields, vets, and assigns requests for community outreach, education, and vaccination clinics, by county- Works with the region outbreak response team to identify and coordinate sites for mobile vaccination clinics- Serves as a lead community representative by attending community coalition and workgroup meetings, making presentations on behalf of the programs and fostering collaborative relationships with community partners, providers, and resources- Manages a contract/event log to track participation, outreach activities, and success stories for monthly reporting to the CED and region leadership- Coordinates resource and logistical needs for the outreach team- Other duties as assigned, including collaboration with the CORE health education team as needed.
