Responsible for supervising a team of multidisciplinary Care Specialists, including but not limited to Certified Diabetes Care and Education Specialists, Registered Nurses, Registered Dieticians, and Mental Health Specialists that provide population health and disease management services and interventions through an application (web and phone based)- Provide strategic direction, motivation, mentoring, and performance management both as a group and individually- Act as a subject matter expert and lead on cases where consultation is required in customizing a protocol involving multidisciplinary and clinical interventions. Subject areas include, but are not limited to: nutrition, diabetes education, chronic condition care escalation and self-management- Develop measurement strategy to track and optimize team productivity and effectiveness in collaboration with other team leads and operations leadership- Collaborate continuously with an interprofessional team in care delivery, with a focus on improving measures and outcomes- Assist in the development of program content, workflows and protocols in designated subject areas; document protocols and workflows for operational and clinical efficiency and effectiveness- Review operational problems / policies and recommend solutions and changes to operations and clinical leadership.
