Coding Compliance/Education accuracy of MSDRG, APRDRG and APC assignment.*Confirm appropriate identification, coding and sequencing of pertinent secondary diagnoses and procedures, severity of illness, etc.*Confirm coding of all diagnoses required to validate medical necessity.*Validate the use or nonuse of the query tool.*Responsible for entry into database for tracking and trending.*Coordinate to rebill the account as needed.*Provide coding review summary reports to management as required, including quarterly entity and coder scorecards.*Monitor for trends during review process and advise appropriate manager of trends/patterns reflecting need for individual action plan(s) or section education.*Assist in the development and updating of procedures to maintain standards for correct coding.*Provide input in the development, refinement and implementation of methods and procedures used to complete review and interpret health record documentation to identify pertinent diagnosis/procedures that require code assignment for inpatient records. Accuracy rate 96%, 2.5 records/hour*Code a variety of inpatient charts from Trauma, Neurosurgery, Long Term Acute Care, Cardiac, Internal Medicine, Obstetrics/Newborn, etc. THR has 1 teaching, two psychiatric and 1 LTAC hospital.*Demonstrate appropriate utilization of coding software (3M, EPIC and Optum 360 CAC)*Query physicians to ensure a complete and accurate medical record.*Correctly identify and abstract all vital abstract data.*Conduct coding reviews on charts where CDI and the coder\'s DRG do not match. Confirm the most appropriate DRG for the record and supply any coding guidelines or other sources that substantiate my opinion.*Started working for THR at Harris Methodist Hospital in Fort Worth - a 726 bed facility.*Centralization of Health Information Services department which includes 14 acute-care hospitals with approximately 3,700 beds.
