As IIH Qualified Professional, treat children, teens and families with mental health diagnoses in variety of clinical settings including home, school and community; Prepare Person-Centered Plans (PCPs), collaborate with internal and external colleagues such as DJJ and DSS, via treatment team meetings, family therapy sessions, and child-family team meetings (CFTs); execute STR’S, transitional discharge plans, TAR Authorizations, ASAM and diagnostic assessments. QP attending court juvenile sessions with proper documentation noted on behaviors of Daymark Recovery. As QP have ability to quickly assess the risks and safety of the client environment during daylight hours, after dark and in high crime areas. Also counseling children and adults in a variety of clinical milieus, including home-based therapy and outpatient MH/SA clinic.
