ABOUT TARA HALLAM
I am a highly motivated, enthusiastic and experienced Social Worker seeking new opportunities. I graduated with a Masters (MA with Merit) in Social Work from the University of Manchester (Dec 2015). I am registered with Social Work England. I am a member of association of palliative care social workers. I work as a Specialist Social Worker at Liverpool Heart and Chest Hospital. It is a regional specialist \'outstanding hospital\' and serves patients from the UK.My role includes and is not exclusive to: I am a Specialist Palliative Care Social Worker, Specialist Cystic Fibrosis Social Worker and Specialist Adult Congenital Heart Disease Social Worker. I also support patients with Heart Failure, cardio oncology and Lung Cancer and Mesothelioma Diagnosis and support hospital discharge and safeguarding. I am an inclusion champion for Liverpool city Council I have previous statutory experience for Cheshire West and Chester Council working in hospital discharge and community case management. Other experience includes working in an adults learning disability team for Bolton Council, as a student social worker. I have also worked directly with adults with learning disabilities in a day service setting for Wigan Council.I have excellent interpersonal and communication, IT and administrative skills. I have experience in case management, strengths based, solution focused, trauma informed, systemic practitioner, holistic assessment and review of complex long term work in a holistic and person centred way with vulnerable adults and their families. I can generate my assessment and review work utilising IT systems. I have excellent advocacy skills and have chaired and represented in Best Interest and CHC meetings and MDT meetings. I am a Practice Educator and passionate about social work education. I produce high quality reports and manage and prioritise my workload effectively. As a law graduate, I can understand and interpret legislation pertaining to adult and children\'s social care and can apply it to the wider needs of the service user and their carers/families. I can make informed decisions and evaluate outcomes in a timely and financially robust way. I can demonstrate anti-discriminatory practice in a social care setting.
