ABOUT TANYA MCADAMS NP-PHC

As the Managing Director at Bayshore HealthCare for the Integrated Care Solutions division, with over 18 years of experience in Nursing and HealthCare, I have developed and implemented inter-professional models of care that are patient-oriented, resourceful, and evidence-based. I have a Master of Science in Aging and Health, a Nurse Practitioner certificate from Queen\'s University, as well as a Bachelor of Science in Nursing from Brock University. I have also completed quality improvement training programs, such as IDEAS and Instincts at Work, and I am an active volunteer board member for an Ontario based NPLC, and most recently a small bed Ontario Hospice. I am passionate about improving care transitions and outcomes for patients and families across sectors and settings. I am a vibrant and story telling keynote speaker, and represented a Canadian Nurse experience of Medical Assistance in Dying internationally in Australia.
