Contribute to the assessment of need for each induvial ensuring that their care needs are met under the Care act 2014 by meeting with patients as well as relevant family members to discuss their needs and wants from the care.• Assist in developing personalized care plan, as well as monitoring and adjusting them when needed to ensure that the services are person centred.• Ensuring that the service users are treated with dignity and respect and report any changes to the service user’s presentation that my warrant a mental capacity assessment, mental health assessments as well as highlighting any safeguarding issues that may arise, there by ensuring that the Mental Health Capacity Act is adhered to.• Evaluating interventions and care plans with the care staff, including identifying areas where further care may be required and reporting back to the Manager.• Support service users to seek appropriate support e.g. crisis support, wellbeing support as well as liaising with professionals where the service user is unable to do so and contribute to the assessments that may be conducted to ensure a holistic approach is given to get the best outcome for the service user.• Work with other professionals such as nurses, Doctors, occupational therapists, and physiotherapists to ensure that the service user is getting the support they need.• Keeping families updated about their relative’s progress and highlighting any concerns so that support can be given without any delay.• Recording and keep up to date records on service users.• Attending team meetings and contributing to the learning of others.• Supporting care workers to ensure that they are completing their duties diligently• Performing monthly audits and spot checks for staff.
