Admissions, and 5. discharge planning.• Coordinated effort to improve patient flow for outpatient and inpatient services including the external referral management process; ED to admitting process; floor-nurse patient receiving process; patient discharge process dependent on lab orders, home oxygen needs; readiness for rehabilitation services; and arranging for charity medications; and claims submission and denials processes – conducted interviews and unit tours; created current and future state process maps.• Facilitated sepsis care performance improvement project to move SEP-1 measure compliance from 24% to 60% in three months (including awareness campaign).• Launched Good Catch and Just Culture programs to encourage incident reporting and learning from near misses.
