My team and I are responsible for commissioning a number of support mechanisms and services and creating change across Adult Social Care in Essex.Some of the highlights include:Developing the personalisation offer across Adult Social Care including: Improving our Direct Payment offer Introducing ISFs Improving access to PAs Generating a micro-enterprise market implementing \'Making it Real\'Commissioning a sensory rehab and reablement service. Coproducing a physical impairment/disability strategy to shape our support and service offer to people with a physical disabilityCommissioned a new model for Coproduction across the council to ensure people and their lived experience is at the heart of all we do.Worked with District, Borough and City Councils to improve the delivery of Disabled Facilities Grant including creating new posts of Senior OTs in housing departments to reduce delayed transfers from hospital and improve the outcomes for people with life limiting or changing conditions.
