As a clinical MSW intern at Elderhaus PACE (Program of All-Inclusive Care for the Elderly), I provided comprehensive psychosocial support to older adults with complex medical and behavioral health needs. Under the supervision of a licensed clinical social worker, I engaged in person-centered care planning, conducted biopsychosocial assessments, and participated in interdisciplinary team meetings to advocate for the holistic well-being of participants. My role included facilitating care coordination, crisis intervention, advance care planning, and connecting individuals and caregivers with community resources. I also gained experience in documentation using electronic health records and deepened my understanding of aging, chronic illness, dementia care, and the PACE model of wraparound services.
