In alignment with Baptist Health System\'s mission to help people achieve health for life through compassionate service inspired by faith, functions as lead provider in newly created disease management program empowered to reduce heart failure readmissions. This role integrates inpatient consultations, outpatient management, education, and community awareness in collaboration with hospitalists, primary care providers, cardiologists, various specialists, nursing staff, and hospital administration to improve outcomes in patients with varying degrees of heart failure. A holistic approach to patient care is taken to include shared decision making and a comprehensive evaluation of social determinants of health with a goal of improving quality of life and survival.
