As a Regional Primary Care Coordinator, I support the Northern Primary Care team in delivering culturally safe and high-quality primary care services to First Nations communities. My role focuses on providing operational and administrative support for the coordination and implementation of primary care initiatives.Key responsibilities include: • Assisting with the planning, development, and implementation of primary care programs and services. • Supporting the coordination of Primary Care Networks and integration initiatives with health partners such as Divisions of Family Practice and the Northern Health Authority. • Managing administrative tasks including meeting coordination, document preparation, tracking project activities, and supporting reporting requirements. • Assisting with needs assessments, data collection, and preparation of updates and briefing materials. • Supporting quality improvement processes and participating in initiatives that promote community-driven, culturally appropriate care.This position plays a key role in enhancing collaboration, streamlining primary care services, and contributing to the overall wellness and health outcomes of Northern First Nations communities.
