As a Mental Health Support Worker, I was assigned as a key worker for three residents with complex mental health needs — primarily schizophrenia and anxiety disorders. I deliver person-centred, recovery-focused care within a supported living environment, developing tailored care and support plans, fostering independence, and advocating for each resident’s wellbeing through compassionate, evidence-based support.Key Responsibilities:Delivered holistic 1:1 support focusing on emotional stability, medication adherence, and daily living skills.Collaborated with healthcare professionals, including community mental health teams, GPs, and social workers, to review progress and ensure continuity of care.Conducted assessments, updated and implemented individualized care and support plans, and documented all interventions in line with GDPR and organizational policies.Promoted residents’ autonomy through encouragement, empowerment, and engagement in community-based and skill-building activities.Applied motivational interviewing and CBT-informed approaches to help residents manage symptoms, identify triggers, and build effective coping mechanisms.Acted as a mentor for new staff, promoting a culture of empathy, dignity, and professional excellence across the team.Key Achievement / Highlight Project:Led a “Positive Routines & Engagement Initiative” for residents living with schizophrenia, introducing structured creative arts and cognitive stimulation sessions. This initiative enhanced engagement by 40% and led to measurable reductions in anxiety and social withdrawal.
