    Responsible for the development and implementation of the Medication Safety, Opioid Stewardship, and Drug Diversion Programs for all FHCC sites (FHCC Hospital and all oncology clinics)• Partnered with medical staff, clinical pharmacy, and nursing to develop oral chemotherapy workflows to improve safety in the multidisciplinary process; rolled out to all services • Led the development of a process for documenting and notifying pharmacists and providers of Adverse Drug Reactions (ADRs) as well as documentation in a central database to facilitate aggregated analysis• Analyzed paclitaxel reaction rates and implemented pre-medication regimen and nursing administration practice changes to 40+ plans to decrease rates of reactions from 5% to a targeted 3% while increasing cost savings by moving to oral pre-medications• Led an organization wide quality improvement effort to move to normal saline only for central venous catheter flushing and locking instead of heparin to improve patient safety; measured rates of occlusions through use of tPA• Partnered with the Investigational Drug Service in response to protocol deviations to improve workflows, processes, and systems to ensure patient safety in clinical trials• Reviewed CMS, DOH, and TJC medication related regulations including National Patient Safety Goals and led workgroups to achieve compliance • Led the development of the Opioid Stewardship program including a dashboard to track Morphine Equivalent Doses (MED) prescribed, rates of opioid prescribing, average quantity prescribed, etc. with comparison between specialties and service lines• Led the development of the Drug Diversion Prevention Program including workflows, a Diversion Response Team, policies, watchlists, and new auditing programs & data dashboards to identify for red flag activity in nursing staff• Developed fiscal year goals with measurable metrics and measures of success for the Medication Safety and Drug Diversion Prevention Program
