Rotates working on patient care units to serve as a resource for staff, patients, and visitors.• Performs renal drug dosing, IV to PO conversions, pharmacokinetic drug dosing, and therapeutic drug monitoring per protocol.• Provides clinical interventions with timely follow up and accurate documentation in Cerner, Sentri7, and Quantifi.Dispensing and Operational duties:• Checks for accurate filling and compounding of medications by technicians. • Reviews and evaluates medication orders for appropriateness and completeness. • Maintains the hospital formulary by minimizing non-formulary procurements, using approved therapeutic substitutions and promoting rational drug therapy selection.• Proficient in pharmacy automated systems and computer software including but not limited to Cerner, Omnicell, and PharmacyKeeper.• Issues controlled substances to patient care areas and maintains records and security according to law and hospital policy.• Adheres to USP-795, USP-797, and USP-800 guidelines for compounding and handling of non-sterile, sterile, and hazardous products (ex: chemotherapy).• Participates in the implementation of treatment guidelines, protocols, formulary changes, and critical pathways as approved by appropriate committees. • Conducts medication regimen reviews for skilled nursing facility patients as assigned. • Participates in medication use evaluation and quality improvement activities of the department, and reports medication events in Midas.• Participates in the training of pharmacists, physicians, nurses, and other health care professionals, as assigned.
