I’m currently working on a project to improve population health and reduce chronic care costs using data. I build machine learning models in Python to predict hospital readmission and emergency visits, with around 87% accuracy.I design ETL pipelines using Azure Data Factory and SSIS to collect and clean data from systems like EPIC, Facets, and claims databases, and store it in Snowflake. I also use social and behavioral health data to help care teams focus on high-risk patients. To support decision-making, I build interactive dashboards in Tableau to track HEDIS scores, disease trends, and service usage. I follow HIPAA rules closely by using automated encryption and masking for patient data. I also standardize data using ICD-10, CPT, and LOINC codes for accurate reporting.
