Furthermore, the increasing administrative load on clinicians reduces their ability to engage proactively with patients, thereby worsening outcomes and amplifying the financial burden on the healthcare system, a challenge that is likely to escalate in the coming years.To address these issues, we are thrilled to unveil “Carrie,” our pioneering voice-based AI Health Companion for chronic care management and coordination. This cutting-edge platform engages patients through natural, goal-directed conversations via phone, providing personalized care and education between clinical visits. Carrie assists in ensuring medication adherence, monitors for and detects emerging symptoms, and integrates with EHR systems to promptly alert care teams for timely intervention. Initially focusing on heart failure, with plans to expand to other comorbid conditions.Carrie is designed to improve patient outcomes by reducing preventable (re)admissions, boosting care team efficiency, and bridging the gap between clinical visits, ultimately fostering healthier living at home and lowering total care costs.
