Responsible for conducting the national Accreditation Council for Graduate Medical Education’s (ACGME) Clinical Learning Environment Review (CLER) site visits. The CLER program provides formative assessments to sponsoring institutions across the country related to domains critical to organizational effectiveness. These domains include patient safety, health care quality, transitions of care, health care disparities, inter-disciplinary teaming, supervision, well-being and professionalism. The CLER process involves discussions with senior leadership, quality and safety leaders, nurses, physicians and other members of the health care team. The CLER team collects, analyzes and presents the data and findings to senior leadership. The ACGME’s CLER initiative provides each institution’s senior leadership with valuable information that can be used to enhance operations and ultimately improve patient care and education. National report of findings are periodically published. I have led CLER team visits to scores of institutions across the country. These institutions range from large prestigious centers of excellence to small community hospitals both with diverse patient populations.Reflecting on the impact and importance of CLER, Thomas J. Nasca, MD, MACP, chief executive officer of the ACGME noted that, “ The CLER Pathways to Excellence document marks an important step in the delivery of patient care by providing teaching hospitals, medical centers, and clinics with a framework to create better learning environments, or ‘pathways,’ that advance the learning outcomes of resident and fellow physicians and seek to improve the safety and quality of care that patients receive.”
